In a species where the role of the Painboy already occupies a uniquely feared position — part doctor, part mechanic, part mad scientist — Grotsnik has pushed the boundaries of what is possible so far beyond the norm that he exists in a category entirely his own. His cybork body modifications are legendary throughout the greenskin race, his surgical innovations have been adopted by Painboyz on a thousand different worlds, and his personal kill count from botched operations alone would qualify him as a significant military threat by Imperial standards.

The Painboyz of Orks society serve a function that is equal parts medical professional and mechanical engineer, reflecting the unique biology of the greenskin species. Orks are remarkably resilient creatures whose physiology includes a symbiotic fungal component that grants them extraordinary healing capabilities — an Ork can survive wounds that would kill any human several times over, and their bodies can accept crude cybernetic implants with a success rate that would be the envy of the most advanced Adeptus Mechanicus magos. Painboyz exploit this resilience to the fullest, performing surgical procedures of breathtaking brutality that would be instantly lethal to any other species but which Orks endure with nothing more than vigorous complaints and the occasional retaliatory punch. His patients wake up to find new limbs, additional eyes, weapon systems they never asked for, and occasionally the organs of other Orks that Grotsnik has transplanted for reasons known only to his own fractured psyche.
